Gentle Family Dentistry offers our patient forms online so they can be completed in the convenience of your own home or office. Download the necessary forms, print them out, and fill in the required information. Bring them with you to your appointment.


FINANCIAL POLICY

We firmly believe that financial considerations should not be an obstacle to obtaining the treatment you desire. We are sensitive to the fact that each patient has different needs and have several payment options available so that treatment is accessible to everyone. All financial discussions and arrangements are kept strictly confidential.


PATIENT INFORMATION & CANCELLATION POLICY

Your appointment and dental health is very important to us. We understand that sometimes unexpected delays can occur causing scheduling adjustments. Please understand, when an appointment is made, this time is specifically reserved for you and appointments are limited.

If you need to cancel your appointment we respectfully request at least 24 hours notice. Please be aware that a 24 hours notice is required when cancelling an appointment to allow us time for scheduling another patient. In the event of a missed appointment you may be subject to a fee.

Our Policy

  • Any cancellation made less than 24 hours will result in a cancellation fee.
  • If you are more than 15 minutes late to a scheduled appointment, please note that we may not be able to accommodate you. In this case we will do our best to reschedule you at another time that is convenient.

INSURANCE INFORMATION

Dental insurance is diverse. Some policies cover only a small percentage of fees, while others cover 50 percent or greater. The more you or your employer pays for premiums, the better the coverage, in most cases. Dental insurance can be confusing, but we'll make it easy for you. Simply bring your card or claim into our office. We'll be happy to tell you what your insurance will cover and file your claim for you.
